The file does not contain any malware although just for clarity sake, was this by google play protect or another antivirus? If it was play protect, it's probably because of the self-signing on the apk

No, this only applies to premium choices, premium outfits and hairstyles still cost gems. It is currently impossible to mod premium outfits and hairstyles without breaking mechanics of the game.
If I were to mod them anyway, they would only be visual and would not affect the story, that is, the game would not recognize that you have selected a premium outfit and dialogue would proceed as though you had selected a regular outfit.
